Diseases of the Nose

The Nose is principally affected by what was traditionally called Catarrh, and is equivalent to the Sinusitis and Rhinitis of modern medicine.

The Nose can also be affected by Loss of the Sense of Smell, as well as diseases such as Polyps.

Catarrh, Sinusitis and Rhinitis

Catarrh is acute inflammation of the mucous of the Upper Respiratory Tract, typically including acute rhinitis, acute nasopharyngitis and common cold.

The Western Tradition held this to be caused by ‘distillations’ from ‘vapors’ rising from the Stomach. Despite this seeming to be a foreign concept to TCM, it is actually related to the Lungs (and Upper Respiratory Tract) being the ‘Upper Source of Water’ in TCM fluid metabolism theory.

The primary differential diagnosis is of Hot or Cold types. Hot type will have thicker phlegm that may be yellow, perhaps with other symptoms of heat such as Sore Throat or Fever. Cold type will have excess clear mucus, a pale face and a cold feeling.

Treatment may include clearing Wind-Cold, Wind-Heat, and strengthening the Stomach.

Gargles:
i. from Cold: decoction of myrrh or libani or sandarac
ii. from Heat: Rose water with Camphor, Rose, Pomegranate flower, Coriander water, Poppy water

Fumigation:
i. from Heat: Linseed infused in vinegar and torrefied; Barley and vinegar
ii. from Cold: Storax, Libano, Aloeswood, Sandarac

Special Formula for Catarrh (Sinusitis & Rhinitis):

The following formulas from De Morbis Internis Curandis, Mesue, 1551:

BALLS FOR HOT CATARRH
    Black Pepper
    Red Sandal.                     1 part ea.
    Rose                                 2 parts
    White Sandal                  3 parts
    Camphor                         5 parts
    Armenian Earth              8 parts
Everything except camphor is moistened with alcohol, then triturated with Rose water for 1 week, then add Camphor; form balls with gum arabic and rose water; smell

BALLS FOR COLD CATARRH
    Storax                              5 parts
    Libani
    Mastic                             3 parts
    Sandarac
    Costus
    Spikenard                       2 parts
    Aloeswood                     1 part

PILLS TO HOLD IN THE MOUTH FOR CATARRH
    Starch
    Tragacanth
    Sweet Almond
    Bean
    White Poppy
    Gum Arabic
    Armenian Earth             equal parts
Mix with mucilage of Psyllium and Quince and form small pills to hold in the Mouth.


POWDER FOR CATARRH
    Flowers of Sulphur
    Saltpeter
    Licorice
    Fennel seed                      equal parts
Dose: 1–2 scruples.

TO RESTRAIN THE LEAKAGE AND THICKEN THIN PHLEGM
    Old Conserve of Roses   6 drams
    Diatragacanth Frigidum  2 drams
    Armenian Earth washed
    in Rose water                  2 scruples
With Syrup of dried Roses, make a confection. Dose as much as a small nut.

TROCHES FOR CATARRH
    Frankincense
    Licorice juice                      1 dram ea.
    Opium
    Saffron
    Myrrh                                   1 scruple ea.
Powder and form Troches (or Pills) with Syrup of Poppy.
Dose: ½ dram–2 scruple

PURGE FOR HEAT-TYPE CATARRH
    Rhubarb                               4 scruples
    Yellow Myrobalan *            ½ dram
    Yellow Sandalwood           ½ scruple
* Myrobalan is triturated with Almond oil.
Infuse in Lettuce and Purslane Water, Add Syrup of Rose Solutive and make a potion.           

CONFECTION FOR CATARRH
    Coriander seed                   ½ oz.
    Nutmeg
    Frankincense                      3 drams ea.
    Licorice
    Mastic                                  2 drams ea.
    Cubebs                                1 dram
    Conserve of Roses            1 oz.
Powder, and with Sugar dissolved in Rose water (10 oz.), Form Troches of 3 drams (or without the Sugar, take 1 dram).

UNANI FORMULA
    Violet
    Marshmallow                    7 grams ea.
    Jujubes                              7 in number
Boil in 100 mls water down to half. Take a little with Honey twice daily.

FOR COLD CATARRH.
    Storax
    Cypress nuts
    Mastic
    Frankincense
    Labdanum
    Amber.                           1 dram ea.
    Licorice scraped
    Raisins                           1 ½ drams ea.
Form pills and hold in the mouth. (The Secrets of Alexis, 1615)

TROCHES TO CHEW FOR CATARRH
    Nutmeg                                1 dram
    Mastic
    Gum Arabic                          ½ dram ea.
Powder and form Troches of Rose Water.
This is used for thin Catarrh. They are chewed in the mouth.

TROCHES TO HOLD IN THE MOUTH
    Gum Tragacanth
    Gum Arabic                         2 drams ea.
    Armenian Earth
    Sealed Earth                       1 dram ea.
    Poppy seed
    Licorice juice                      ½ dram ea.
    Sugar penids                      1 oz.
With Mucilage of Quince seed made with Rose water, form Troches to be held in the mouth.

CERATE FOR CATARRH
    Clove
    Cinnamon
    Mastic                              1 ½ drams ea.
    Cypress nuts
    Red Coral
    Pomegranate flower.     6 grains ea.
Powder and with Rosin make a plaster to be applied to teh sutures of the skull and it will giove great relief.
  This is only suitable for Cold and Damp causes. The same types of formula can be taken inwardly. (The Secrets of Alexis, 1615)


Loss of the Sense of Smell

1. Calamus has a reputation for obstruction of Qi by Phlegm and Damp which affects any of the senses, including loss of smell.
2. Pellitory is likewise similar to, but stronger than Calamus
3. Magnolia flower, Xin Yi is used in TCM, and has a fragrant scent which opens the passages of the nose
4. Xanthium, Byi tsher is also useful

In the Western Tradition, purging is usually indicated unless the person is very weak. Purges used for Catarrh, above, may be used.

Topically
Purge the Body; any of the following strong scents can awaken sense of smell:
1. Oregano mixed with Vinegar and Salt
2. Benzoin dissolved in Vinegar
3. Mustard seed mixed with Vinegar
4. Cyclamen juice taken up the nose

Loss of Smell:
“To recover the lost Smell: Take a sufficient quantity of Roman Nigella, reduce it to Powder, and pound it with old oil in a Mortar: Put of this Liquor into your Nose and snuff up the Spirit, holding up your Head in the mean while, and having your Mouth full of Water”. (The Compleat Herbal, Tournefort, 1719)


Nasal Polyps

1. Painting the juice of Garlic has been used.
2. Birthwort and Cyperus cures Nasal Polyps (Pliny)
3. Earthworm “Cures Polyps in the nostrils”. (Li Shi Zhen)
4. Apply Sal ammoniac powder to Nasal Polyps “and they will fall off”. (Ben Cao Gang Mu)
